In order to participate in ANY science lab, experiment, or demonstration, the following safety declaration MUST be completed, signed, and returned to me during the first week of the course.

Step 2: Once you have completed reviewing the safety information provided above, please complete the following safety quiz.
You must receive a grade of 75% or above on the safety quiz.
You have three attempts at the safety quiz.
You must sign-in using your Notre Dame student Google account.
